Final Day to Submit 471 E-rate Applications

A list of important reminders...
** It's too late to submit the 471 VIA MAIL and still be considered in the
window. If you have no choice but to file after tonight's deadline, please
do it as soon as possible, as applications received outside the window are
considered on a first-come, first-served basis if there are funds remaining
after all in-window application have been funded.
** It's NOT too late to file the 471 ELECTRONICALLY. According the SLD's
recent update, you have until 11:59 p.m. tonight to press the submit button,
and then until Wednesday, January 26 to have your signature, certification
page and 471 attachments filed with the SLD. If you still have not mailed
these documents, I strongly urge you to not use the normal postal service as
your mechanism for delivery, as some applicants have found out that the US
Postal Service has not delivered documents to Kansas in a timely manner.
Please use a 2 or 3 day delivery service that provides confirmation of
delivery.
** Don't forget that the 471 application is not considered filed unless they
received your signature, certification and attachments by 11:59 p.m. on
Wednesday, January 26.
** The zip code for Lawrence Kansas that was in a previous e-mail and that
is posted on the SLD web site is incorrect (Thanks to Jeff Matthews at East
Allegheny SD for pointing this out!) The correct address is:

** If you are purchasing from PEPPM, the information you will need in order
to complete the 471, Block 5 is located on the PEPPM web site at
www.peppm.org.
** If you completely forgot to file a 470 for services, you can still
purchase internal connections from the PEPPM contract and file your 471
online by 11:59 p.m. tonight because the 470 for that contract already has
been filed on your behalf.
** If you have selected a service provider, but are having trouble finding
their SPIN, call the SLD at 888-203-8100 and they will provide you with a
tool to complete the online form. If you do not have the SPIN or this tool,
the online application will not let you proceed. Remember that CSB hours of
service are 8:00AM to 8:00 PM, ET.
** If you are filing on behalf of a multi-year contract, or a contract with
renewal options, don't forget to put the ending date as the very last date
that your contract could terminate. So if you have a 2 year contract, with
2 one-year renewal options in it, put the end of year 4 as the contract
expiration date on the 471, Block 5.
** To save time, file just one 471 for all of your contracts, putting each
separate contract on a separate Block 5. This will save you alot of time
when filing online.
** Don't forget to make sure you have checked the correct category of
service on Block 5. Some folks I've talked to just didn't see the default
was checked and submitted it with the wrong category of service. Also,
don't forget that Telecommunciations Services can only be provided by
regulated common carriers.
** If you're not sure if something is ineligible, put it on a separate Block
5. That way if it's ineligible, just that funding request will be
disqualified, not the other Block 5 requests.
